The ability to dream within dreams is based on the fact that that actually does happen sometimes. I think, in-story, The Dreamworld Corp. is a corruption of our natural ability to dream. So some dreams are orchestrated, but we still have the ability to use our subconscious to create a dream. So, basically, everything beyond the 'outmost' layer is her own mind. I think the reason she wasn't dreaming was just because she was very depressed.

Probably just a commander or something, and likely the same one that was perched on top of her house.
